APPLICATION FOR ENROLMENT

Complete the application form online or print a copy, and return as soon
as possible to agovender@fatima.co.za.
Please use the check list below and ensure that all the relevant
documentation is attached prior to submitting the application.
1. Copies of ID documents for both parents/guardians.
2. Copy of child’s birth certificate.
3. Child’s most recent half-year or end-of-year report.
4. Copies of remedial/psychological or educational reports or
assessments.
5. Proof of payment of the application fee of R450 into the following
account:

Account No: 1352 0020 94
Reference: Child’s initial, surname, grade and year of entry
6. Both parents/guardians must sign the application.
